PREPARE FOR YOUR FIRST APPOINTMENT
Once your first appointment at Well Clinic has been confirmed, there are a few essential action items to complete before you arrive.
In your confirmation email, you will find a series of new patient paperwork and forms. Please read, fill out and sign all of them, then fax or e-mail everything back to us prior to your first appointment.

For Those Seeing a Psychiatrist
Please ensure that you have faxed or e-mailed us your paperwork and forms at least 48 hours before your appointment, otherwise it may be cancelled or rescheduled.
This is especially important, as your psychiatrist will spend time preparing for your visit by reviewing your health history, charts and lab results in order to provide you with high quality care starting on day one.
Also, if you have access to any of the below items, please e-mail or fax us your:
- Previous mental health medical records
- Lab results from within the last year
- Psychological test results
Please arrive 15-20 minutes early. If you are driving, giving yourself extra time will allow for parking delays and help keep stress levels at bay.
Downtown: Please check in with the building security guard when you arrive. For sessions beginning at 6:00pm or later, the building attendant will let you in.
Potrero Hill: Refer to your confirmation e-mail for the building security code, then enter on the first floor and walk up the stairs.
